- Government-issued photo ID
- Employer/enrollment proof (for ESIC)
- Referral or authorization letter (if applicable)
- Recent medical records or prescriptions
Cashless Claim Process
- Visit: Come to our claims desk with your documents
- Verification: We check eligibility and scheme coverage
- Pre-authorization: We coordinate with the scheme authority
- Treatment: Cashless care begins once approved
- Discharge: Billing is settled as per scheme policy
